Service Facilitator – Disability Services 

Full-Time – Belmont 12 month contract

Baptistcare’s Disability Services uses a fresh approach, working in partnership with our customers, their families, friends and other stakeholders to find the best way of achieving excellent outcomes for both individuals and families. Disability Services operates in the Perth metro area and select areas of regional WA to assist our customers to live a fulfilling and independent life within the community.

Position Details

Baptistcare is currently seeking a Service Facilitator to join our team. The role of the Service Facilitator is to ensure the provision of high quality, flexible and personalised support that is holistic and enhances the quality of life for service users, in accordance with their own individual outcomes and contractual compliance. Reporting to the Operations Manager, the position serves as the main contact point for service users and their families in regard to the services delivered.

The position implements, monitors and reviews service provision; manages resources and associated reporting; and builds positive relationships within the community to maximise available service delivery options for service users.

Essential Criteria

  • Tertiary qualification in Social Work, Occupational Therapy, Psychology, or a related human services degree and or extensive experience in human services.
  • Effectively able to lead teams and individuals, with an appreciation and demonstrated capacity around collaboration, accountability and equity.
  • Considerable experience and knowledge around facilitating complex services for people with disabilities and/or mental health.
  • High level conceptual and analytical skills to assess and develop individual goals of care that meet contractual compliance and are person-centred.
  • Highly developed communication skills (both oral and written) and interpersonal skills including the ability to work with the client group and colleagues.
  • Demonstrated ability to network and engage with the community and other stakeholders to develop effective relationships.
  • Broad knowledge of financial processes and guidelines related to individual funding and budgets including knowledge and experience of NDIS services
  • Sound organisational skills with the ability to work independently, meet deadlines and manage competing priorities.
  • Competence in using a variety of IT including complex databases and client management systems.

Desirable

  • Knowledge of contemporary psychosocial rehabilitation and recovery principles and strategies associated with individuals with mental health issues.
  • Knowledge of current issues impacting on people with disabilities.
  • Understanding of continuous quality improvement principles and participation in quality activities.
